FACILITIES CHANGE REQUEST

Submit a Facilities Change Request for proposed alterations to a space on campus, including offices and classrooms. It may include new furniture or furniture adjustments, as well as adjustments to the actual facility.

Please submit a work order for single furniture items, or for any repairs.

1. Develop Scope and Cost Estimate

Facilities Management will assist in the development of your scope and cost estimate. In these preliminary stages, a general scope and cost estimate will be created. To set up a meeting and a walk-through, please email the Campus Space Coordinator at curt.christiansen@byuh.edu

2. Fill Out Request for Facilities Change Form

With your general scope and pricing in hand, you can now fill out the Facilities Change Request Form. The following are the main sections of the form:

  • Justification of Request
    Please provide sufficient detail to describe the project being requested. Include details on how this change will impact students, the mission of the university, the existing space, and any external impacts it may have on the community.
  • Estimate for Funding
    Include any department funding that is available for the project, and the amount you are requesting from Campus Planning Committee Funds.
  • Endorsement Statement and Signatures
    Obtain an endorsement and signature from your Dean or Department Head. Your Dean or Department Head will forward the form to their respective Vice President for their consideration, endorsement, and signature.

3. Submit Online Request

Submit your request. You will need to attach the documents that were prepared in steps 1 and 2.

  • Scope and Cost Estimate
  • Request for Facilities Change Form

4. Review by the President's Council

The President's Council will review your request. They will consider the needs for space, facilities improvements, and capital projects that align with the university's priorities. You will receive notice from Facilities Management if your request is approved or disapproved.

  • The President has full authority over space categories and classifications.
  • The President's Council is responsible for making changes to space categories and allocation on campus and reviewing projects for the annual capital improvement and capital development budget process.

5. Authorization to Proceed

If your request is approved, Facilities Management will work closely with you to further develop your design and scope, as they move forward with the project.

  • Design will commence with a more detailed scope of the project and a refined Total Project Cost Estimate Budget.
  • The project will be scheduled for bidding when the design is completed and the funding is secured.
